Description

http {
...
deny 1.1.1.1;
...
server {
server_name example.com;
...
set_real_ip_from 1.1.1.1;
real_ip_header X-Forwarded-For;
...
}
}

If I send request like
curl -I -H 'X-Forwarded-For: 2.2.2.2' example.com
I get
HTTP/1.1 200 OK

but if I send
curl -I example.com

HTTP/1.1 403 Forbidden

It seems that realip_module runs before access_module and after change $remote_addr access_module works with changed $remote_addr, but not with $realip_remote_addr as expected.

This is a bug or feature?

This is how it is expected to work. The set_real_ip_from directive changes the client address for all places where the client address is used, including the access module. It is expected to be used to provide a real client address from a trusted frontend server.
